Hands of Hope (Reg. No. R/2377/K266), Inigala, Katugastota, collects household information only to identify and support families in need. Participation is voluntary. Read the full terms. The agreement is available after you reach the end.
1. Purpose of collection
The information you provide is used only to assess eligibility for charitable assistance, plan and deliver aid programmes, and maintain records required for the lawful administration of these programmes. It is not used for any commercial purpose.
1. එකතු කිරීමේ අරමුණ
ඔබ ලබා දෙන තොරතුරු භාවිතා කරන්නේ පුණ්යාධාර සඳහා සුදුසුකම තක්සේරු කිරීමට, ආධාර වැඩසටහන් සැලසුම් කර ලබා දීමට, සහ එම වැඩසටහන් නීත්යානුකූලව පවත්වාගෙන යාමට අවශ්ය වාර්තා තබා ගැනීමට පමණි. එය කිසිදු වාණිජ අරමුණක් සඳහා භාවිතා නොවේ.
1. சேகரிப்பின் நோக்கம்
நீங்கள் வழங்கும் தகவல்கள் தொண்டு உதவிக்கான தகுதியை மதிப்பிடவும், உதவித் திட்டங்களை வழங்கவும், அந்தத் திட்டங்களைச் சட்டப்படி நிர்வகிக்கத் தேவையான பதிவுகளை வைத்திருக்கவும் மட்டுமே பயன்படுத்தப்படும். வணிக நோக்கத்திற்குப் பயன்படுத்தப்படாது.
2. Information we collect
This includes names, contact details, National Identity Card numbers, address, household composition, income range, education, occupation, and health or vulnerability indicators. You may decline to answer any question. Some assistance may depend on the information available.

2. நாம் சேகரிக்கும் தகவல்
பெயர், தொடர்பு விவரங்கள், தேசிய அடையாள அட்டை எண், முகவரி, குடும்ப அமைப்பு, வருமான வரம்பு, கல்வி, தொழில், மற்றும் சுகாதார அல்லது பாதிப்புக் குறிகாட்டிகள் இதில் அடங்கும். எந்தக் கேள்விக்கும் பதிலளிக்க மறுக்கலாம். சில உதவிகள் கிடைக்கும் தகவலைப் பொறுத்தது.
3. Lawful basis and consent
We process your personal data on the basis of your consent and in accordance with the Personal Data Protection Act No. 9 of 2022 of Sri Lanka. You may withdraw your consent at any time without affecting the lawfulness of processing already carried out.

3. சட்டப்பூர்வ அடிப்படை மற்றும் ஒப்புதல்
இலங்கையின் 2022 ஆம் ஆண்டின் 9 ஆம் இலக்க தனிப்பட்ட தரவு பாதுகாப்புச் சட்டத்தின்படி, உங்கள் ஒப்புதலின் அடிப்படையில் தரவு கையாளப்படுகிறது. ஏற்கெனவே செய்யப்பட்ட செயலாக்கத்தின் சட்டப்பூர்வத்தன்மையைப் பாதிக்காமல், எப்போது வேண்டுமானாலும் ஒப்புதலைத் திரும்பப் பெறலாம்.
4. Storage and security
Records are stored securely, with access limited to authorised personnel of Hands of Hope. We apply reasonable organisational and technical safeguards to protect data against loss, misuse, or unauthorised access.

වාර්තා ආරක්ෂිතව තබා ඇත. ප්රවේශය Hands of Hope හි බලයලත් නිලධාරීන්ට සීමා වේ. අලාභය, අවභාවිතය, හෝ අනවසර ප්රවේශයෙන් ආරක්ෂා කිරීමට සාධාරණ පියවර ගනු ලැබේ.
4. சேமிப்பு மற்றும் பாதுகாப்பு
பதிவுகள் பாதுகாப்பாக வைக்கப்படும். அணுகல் Hands of Hope இன் அங்கீகரிக்கப்பட்ட ஊழியர்களுக்கு மட்டும். இழப்பு, தவறான பயன்பாடு, அல்லது அனுமதியற்ற அணுகலிலிருந்து பாதுகாக்க நியாயமான நடவடிக்கைகள் எடுக்கப்படும்.
5. Sharing and disclosure
We do not sell or rent your data. It may be shared only with partner relief organisations, donors, or government authorities, strictly for aid delivery, or where disclosure is required by law. Any sharing is limited to what is necessary.

5. பகிர்வு மற்றும் வெளியீடு
உங்கள் தரவு விற்கப்படவோ வாடகைக்கு விடப்படவோ மாட்டாது. உதவி வழங்குவதற்குத் தேவையான அளவில் மட்டுமே கூட்டாளர் நிறுவனங்கள், நன்கொடையாளர்கள், அல்லது அரசு அதிகாரிகளுடன் பகிரப்படலாம். சட்டம் கோரினால் வெளியிடப்படலாம்.
6. Retention and your rights
Data is retained only as long as necessary for the purposes above. You have the right to access, correct, or request deletion of your data, and to withdraw consent, by contacting info@hohinigala.org or +94 77 5454 144.

Consent
I, the head of household, confirm that I have read and understood the terms above. The information I provide is true to the best of my knowledge. I voluntarily consent to Hands of Hope collecting and processing this information for the purposes stated above.
